Description

This data file contains the Usual, Customary and Reasonable (UCR) fees by state for the entire United States. Medical, Anesthesia, Ambulatory Surgical Center, HCPCS and Dental fees are available either separately or as a package. Data files are available by state or for the entire United States. This is a table driven system designed to eliminate redundancy and provide additional versatility and accuracy. Updated annually in January.
