The Injections Fee Schedule contains the complete listing for injectable drug fees effective January 1, 2004. The Injections Fee Schedule contains the carrier assigned values for HCPCS "J" codes and CPT^© immunization injections, if assigned, on a per carrier basis. Each unique injections schedule provides HCPCS "J" code values missing from RBRVS fee schedules. This data is provided for the entire United States.
