Record Layout - Fixed Field

Fields	Columns			Description
1	1	-	6	lCD-9-CM Code Number (with periods)
2	7	-	11	ICD-9-CM Code Number (without periods)
3			12	Asterisk (*) indicates not the most descriptive code available
4			13	Sequence letter (A,B,C,etc.)
5	14	-	80	Long Description fragment

Note: If a code description is longer than 67 characters, it has been broken into multiple records marked with a sequence letter. The longest description is under 250 characters in length. Therefore, no more than 5 fragments are ever used to supply a particular ICD-9-CM Long Description.
